Csiza Health Care brings the visit to you, with the exception of DOT physicals.
The type of practice has several benefits to it. First, it allows the patient to be seen in the comfort of their own home. This is a big bonus when you are not feeling well and leaving home to get treated is the last thing you feel like doing, especially on those cold snowy or rainy days!
Second, it provides the patient with a comfortable environment that they know. Often patients are nervous about seeing a provider in an unfamiliar environment which leads to increased anxiety for the patient. Being in their own home allows them to feel more in control and relaxed.
Third, it is convenient for patients that cannot drive or do not have reliable transportation to get them to an appointment.
It is especially helpful for a parent with multiple children because you don’t have to pack everyone up and drag them with you. They can stay and play in the comfort of their own home.
Fourth, we don’t pay rent on an office. This reduction in overhead allows us to offer low cash pay prices for certain tests and procedures and sliding scale visit fees for patients that are
underinsured, uninsured, or CHC is out of their insurance network.
Fifth, having the clinician visit the patient in their own home can be very helpful to diagnosis and treatment. Being able to see the surroundings in which the patient lives can offer insight to the provider as to what might be happening and how to best treat the patient.
Our Family Nurse Practitioner is Nicole Csiza.
Nicole has been a nurse since 2008. She has worked in multiple specialties as a registered nurse including long term care and memory care,medical-surgical, orthopedics, preoperative and postoperative, operating room, mom/baby, and
neurology.
She has also worked in the outpatient ambulatory setting of a primary care office.
Nicole became a Family Nurse Practitioner in 2017. During her advanced training she completed two rotations in primary care, two rotations in urgent care/ occupational health, and one rotation in women’s health care.
She is very experienced with the elderly population as she has worked in home health since becoming a NP. She is experienced with dealing with complex patients with multiple comorbidities. Nicole enjoys the challenge balancing complex patients brings. However, she enjoys seeing patients of all ages with any range of health needs.
Nicole has been married since 2006. Her husband is a pilot which has allowed her to work coast to coast. She has three beautiful children, two girls and a boy. They love going apple picking or
whatever fruit is in season!

B12, is a vitamin found in food and is also used as a dietary supplement. Vitamin B12 helps to utilize fats and carbohydrates for energy and make new proteins.
